I understand that the HSCC, the CCACF and PS house stray and unwanted animals. Because many of these animals come from unknown backgrounds, the HSCC and CCACF make no claims or representations regarding the health, temperament or disposition of the homeless animals. I understand that these homeless animals may be unpredictable in their behavior and may bite, scratch or otherwise cause harm whether intentionally or unintentionally and also that the status of the health and vaccinations (including rabies) is often unknown.

I understand that all information such as the names, address and phone numbers of owner surrenders, adopters, and cruelty cases are to be kept private and confidential. This information is not to be released to the general public, or given to potential adopters.

In signing this agreement, I verify that I understand and assume the risks involved in volunteering with the HSCC at the CCACF, the Lynchburg PS store and any and all other locations.  I hereby release HSCC, the CCACF, and PS, their representative, officers, members, employees, and agents from any responsibility for any and all injuries, property damage, all actions, causes of actions, claims, demands, costs, expenses and compensation, on account of, or in any way growing out of my volunteer activities with the HSCC. I agree to indemnify and hold harmless the HSCC as well as the CCACF and PS, their representatives, officers, members, employees and agents from any and all liability arising out of any damages caused by my volunteer activities with the HSCC.  This waiver does include myself, all of my family members and descendants forever from seeking any legal action against the HSCC, the CCACF, and PS or their representatives.

I have read all of the above, understanding and assuming the risk, and agree to be legally bound by the terms and conditions stated.
